摘要
目的观察氟西汀合并小剂量奥氮平(剂量≤5mg/d)治疗抑郁症的临床疗效。方法将60例抑郁症患者随机分成氟西汀组(单用组)和氟西汀合用奥氮平组(合用组),共观察8周,于治疗前及治疗后1,2,4,6,8周末采用汉密尔顿抑郁量表(HAMD)、副反应量表(TESS)评定疗效及副反应,并随访1年观察其复发率。结果合用组与单用组的显效率分别为80.00%和70.00%,差异有显著性(P<0.05);两组治疗后1,2,4周末HAMD评分与治疗前比较,差异均有显著性(P<0.05);两组的HAMD因子分在焦虑/躯体化和睡眠障碍方面差异有显著性(P<0.05),TESS评分及1年内复发率差异无显著性(P<0.05)。结论氟西汀合并小剂量奥氮平治疗抑郁症起效快,可提高疗效,并能迅速改善睡眠障碍和焦虑/躯体化症状,对复发率无明显影响。
Objective To study the clinical effect of fluoxetine combined with small- dose olanzapine （≤ 5 mg/d） in the treatment of depression. Methods 60 hospitalized patients met CCMD-3 criteria of depression were randomly divided into the search （fluoxetine and olanzapine） group and the control （fluoxetine） group for the treatment of 8 weeks. The efficacy and side effects were evaluated with HAMD and TESS. To observe the relapse of one year by follow up. Results The treatment of fluoxetine combined with olanzapine was effentive and the tolerance. There were significant differences in the recovery rate,HAMD of 1, 2,4 week and HAMD factor score in symptoms of body discomfort, sleep disorder and orexis after treatment between two groups. But there was no difference in TESS and the relapse of one year between two groups. Conclusion Fluoxetine combined with small- dose olanzapine can improve the efficacy compared with fluoxetine in treating depression,and improve sleep orexis and the feeling of body discomfort without distinct effect on relapse.
