摘要
目的:探讨异位胸腺瘤的临床特点、诊断、病理、治疗及预后。方法:对3例异位胸腺瘤及文献资料进行回顾性分析。结果:异位胸腺瘤常见于中后纵隔、心包、颈部、甲状腺、肺实质、胸膜及胸壁等部位,主要症状为咳嗽、胸痛及局部压迫症状,肿块多边缘清楚,密度均匀,可见钙化,直径多大于5cm。细针抽吸细胞学检查易误诊,术前诊断困难。胸腺瘤镜下由不同比例瘤样上皮细胞及淋巴细胞混合组成,光镜及免疫组化分析可确诊,外科手术是其主要治疗手段。结论:异位胸腺瘤术前诊断困难,外科手术是其主要的诊断和治疗手段,预后与病理类型有关。
Objective： To explore the clinical pictures, diagnosis, treatment and prognosis of ectopic thymoma. Methods： Three cases with ectopic thymoma were analyzed retrospectively, and related literatures were reviewed to analyze their clinical pictures, diagnosis, treatment and prognosis. Results： Ectopic thymorna is a rare tumor, and mainly composed of a mixture of epithelial cells and lymphocytes. Ectopic thymomas mostly occurred in the neck, middle and posterior mediastinum, intrapericardial sac and the lung, pleura and chest wall. Misdiagnosis often occurred by.fine needle aspiration （FNA） cytology. The surgery is the first choice treatment. Conclusion： Ectopic thymoma is easy to be misdiagnosed, and surgical resection is the main method of diagnosis and treatment, and prognosis is determined by its pathologic classification.
