Genistein预处理对体外循环MIRI的保护作用

摘要 目的通过建立猪体外循环(cardiopulmonary bypass,CPB)心肌缺血再灌注损伤(MI/RI)模型,探讨染料木黄酮(Genistein)预处理对CPB MI/RI损伤后时相的心肌保护作用。方法将16只成年家猪随机分为对照组(Con)和实验组(Gen)(n=8)。常规建立CPB模型,主动脉阻断同时灌注改良St.Thomas停搏液。每组于CPB前、CPB结束时、主动脉开放后2 h检测冠状静脉窦血肌钙蛋白(TnT)含量,外周血中IL-6、IL-8、TNF-α浓度;并于相应时间点监测心功能变化;分别于CPB前和主动脉开放后2 h取左心室心肌,进行心肌组织cNOSi、NOS活性测定。结果(1)和对照组相比,实验组心功能指标明显改善(P<0.05);(2)对照组的TnT以及炎症因子明显高于实验组(P<0.05)。(3)和对照组相比,实验组iNOS和cNOS活性显著增高(P<0.05)。结论CPB前Genistein预处理能够引发心肌缺血再灌注损伤模型的后时相心肌保护作用,其中cNOSi、NOS扮演了重要角色。 Objective By establishing the swine cardiopulmonary bypass(CPB) myocardial ischemia-reperfusion (MI/R) model, the late phase cardioprotective effects were studied. Methods 16 adult swines were randomly divided into 2 groups( n = 8):control group(Con)and experiment group(Gen). CPB was instituted routinely and the improved St. Thomas cold cardioplegic solution was administered into the root of aorta. Blood samples were taken from coronary sinus before CPB,the end of CPB and two hours after the reperfusion to measure TnT . The peripheral blood were taken at the same time points to measure the concentration of IL-6, IL-8 and TNF-α. The LVEDP and ± dp/± dtrnax were measured at the corresponding time points. Specimens of the left ventricular myocardium were taken before CPB and two hours after reperfusion for activity of NOS(iNOS,cNOS). Results (1)Parameters of cardiac function in control group were found decreased than in Gen(P 〈 0.05 );(2) Concentrations of TnT and inflamation factors increased in control group compared with Gen( P 〈0.05). (3)The activity of NOS in Gen were higher than those in control group. Conclusion Cardioprotection were induced by Genistein Preconditioning after Ischemia- Reperfusion during cardiopulmonary bypass in swine, eNOS and iNOS play important roles in cardioprotection.
