摘要
背景与目的:常规影像技术(CT、MRI和DSA)难以完整显示蝶骨嵴脑膜瘤同颅底血管、骨质三者之间关系,对临床处理蝶骨嵴巨大脑膜瘤指导意义有限。本研究旨在总结3D-CTA显示蝶骨嵴脑膜瘤同颅底血管、骨质三者关系的影像特点,探讨其对蝶骨嵴脑膜瘤治疗的指导意义。方法:本文对7例蝶骨嵴脑膜瘤的CTA检查结果进行分析,重建肿瘤与颅底骨质及血管的图像,评价64层3D-CTA在制定蝶骨嵴脑膜瘤治疗方案的价值。结果:64层3D-CTA突破传统影像学上二维平面图像的局限,其VR技术容许骨质的虚拟切除和图像的360°全方位旋转,可以从不同角度观察肿瘤、血管和颅底骨质三者关系,并根据立体影像模拟使用不同入路暴露肿瘤,还可通过调整CT显示阈值揭示包绕于肿瘤中央的颈内动脉及其分支。我们依据患者CTA结果确定方案采取治疗,均获得满意效果。结论:3D-CTA在显示蝶骨嵴脑膜瘤与周围骨质、颅底大动脉及其分支的关系上明显优于CT、MRI、DSA;64层3D-CTA立体影像对制定蝶骨嵴脑膜瘤治疗方案尤其是手术计划有指导意义。
BACKGROUND & OBJECTIVE:Conventional imaging techniques such as computed tomography(CT), magnetic resonance angiography (MRA) and digital substraction angiography (DSA) have many difficulties in demonstrating the three-dimentional relationship between sphenoid ridge meningiomas, cerebral vessels and skull base. In this study, we evaluated the value of 64-slice Spiral three-dimensional CT(3D-CTA) in the management of sphenoid ridge meningiomas. METHODS: 3D-CTA image data of 7 patients with sphenoid ridge meningiomas were reviewed. The value of 3D-CTA in the treatment of sphenoid ridge meningiomas was evaluated. RESULTS: 64 slice 3D-CTA allows virtual bone-removal and omnibearing revolve . It showed the location of basion, vessel and tumor from different views. All seven patients were treated according to the surgical plan which based on the CTA images. The outcome was very good. CONCLUSION: Sixty-four-slice spiral 3D-CTA can demonstrate the three-dimentional relationship between sphenoid ridge meningiomas, cerebral vessels and skull base more accurately than conventional imaging techniques. It is invaluable in planning and the surgical management of sphenoid ridge meningiomas.
