胆囊癌的CT诊断

CT diagnosis of gallbladder carcinoma

目的探讨CT对胆囊癌的诊断价值及CT表现。方法收集1998～2005年间经CT检查及手术病理证实的10例胆囊癌进行回顾性分析。结果壁厚型5例,肿块型3例,腔内型2例。肝脏受侵犯4例,淋巴结转移4例,肝内、外胆管扩张4例,腹水2例,5例合并胆囊(管)结石。结论CT在胆囊癌的诊断中占重要地位,它能发现和显示病变,确定病变侵犯范围及转移情况,对确定肿瘤术前分期,选择治疗方法及判定预后起着重要作用。但在胆囊癌的早期诊断中则有一定限度,尤其是发生于腔肉的微小病变与息内难以鉴别,应密切结合临床及B超检查。

Objective To explore the CT findings and value in the diagnosis of gallbladder carcinoma. Methods CT findings in 10 patients with pathologically-proved gallbladder carcinoma were retrospectively analyzed. Results Of 10 patients, cystic wall thickening was found in 4 cases, soft-tissue mass in 3, and nodules within gallbladder in 2. Conclusion CT played an important role in the diagnosis of gallbladder carcinoma. It can detect lesions, defining infiltrating range of the lesions and metastasis. It was also of great importance in defining tumor staging before operation, therapeutic selection and judging prognosis. However, it had some limitation in early diagnose of gallbladder carcinoma.
