运动区皮质刺激治疗顽固性丘脑痛的初步研究

Motor cortex stimulation for intractable thalamic pain:a preliminary study

目的探讨运动区皮质刺激治疗顽固性丘脑痛的手术方法及效果。方法以条形电极对3例顽固性丘脑痛患者行对侧运动区皮质刺激并植入电子刺激器进行慢性电刺激。结果3例患者术后症状明显缓解,疼痛视觉模拟评分(VAS评分)较术前显著下降,无偏瘫、癫痫等并发症发生。结论运动区皮质刺激是控制顽固性丘脑痛的有效方法,中央沟精确定位及调节合适的刺激参数尤为重要。

Objective To explore the methods and operative effect of motor cortex stimulation for treating intractable thalamic pain. Methods We placed strip electrodes on the surface of precentral gyrus in three patients suffered with intractable thalamic pain, in whom an electrical impulsator had been implanted for giving chronic electrical stimulation. Results Satisfactory effect had been achieved in three patients and visual analogue scale（VAS） scores were decreased remarkably after operation. There was no complication happened. Condusion Motor cortex stimulation is an effective therapy for intractable thalamic pain. It is very important to accurately determine the position of central sulcus and to adjust proper stimulating parameters.
