抗甲状腺药物致肝损伤31例相关因素分析 被引量:3

摘要 目的:分析抗甲状腺药物(ATD)引起肝功能损伤的临床特点以及相关因素。方法:对31例因服用ATD引起肝功能损伤的甲状腺功能亢进症(甲亢)患者的诊断、临床特征、用药、相关因素及预后恢复进行回顾性分析。结果:31例肝损伤患者中,初次用药者发生肝损伤时间为4~115d,肝损伤发生于3个月内者26例(84%)。他巴唑和丙硫氧嘧啶导致肝损伤组与无肝损组在性别、年龄、甲亢病程、甲亢合并显著甲状腺肿大、治疗前碱性磷酸酶及胆红素水平方面无统计学差异,他巴唑致肝损伤组与无肝损组在治疗前甲状腺激素水平、甲亢合并显著突眼及合并甲亢性心脏病比较有统计学差异(P<0.05),31例患者经停药及治疗后,肝功能完全恢复时间为7~96(41±23)d。结论:对于重度甲亢患者及伴有显著突眼、甲亢性心脏病的患者服用他巴唑更易导致药物性肝损伤,肝损伤多发生在初次服药后3个月内;国际共识会议药物性肝损伤诊断标准能够有效地诊断、鉴别药物性肝损伤。
