微淋巴管生成在大肠癌转移中的作用

摘要 目的探讨微淋巴管形成在大肠癌转移中的作用。方法采用体视学原理切片定量分析测定大肠癌Dukes A期、B期、C期、D期各20例及有、无淋巴结转移组各40例组织标本微淋巴管密度和最多截面平均面积。结果大肠癌DukesA期、B期、C期、D期微淋巴管密度值分别为:(1.37±0.25)个/mm2、(1.49±0.31)个/mm2,(2.61±0.58)个/mm2,(2.87±0.57)个/mm2,4组经秩和检验,差异有统计学意义(P<0.05)。大肠癌DukesA期、B期、C期、D期组织最多截面平均面积分别为(9.67±1.14)μm2(、10.62±2.22)μm2(、18.46±4.15)μm2、(20.30±4.07)μm2,4组经秩和检验,差异有统计学意义(P<0.05)。无淋巴结转移组(DukesA期、B期)微淋巴管密度值为(1.43±0.28)个/mm2,最多截面平均面积为(10.15±1.41)μm2,有淋巴结转移组(DukesC期、D期)微淋巴管密度值为(2.74±0.58)个/mm2,最多截面平均面积为(19.38±4.16)μm2,两组比较,差异均有统计学意义(P均<0.01)。结论大肠癌患者中随着病程的发展,肿瘤临床分期越晚其微淋巴管密度值、最多截面平均面积增大,提示微淋巴管生成在大肠癌转移中起着重要作用。 Objective To examine the role of microlymphatical formation in colorectal carcinoma metastasis. Methods The stereological method was used to assay the lymphatic microvessel density, maximal profile average area of Dukes A phase 20 pou, DukesB phase 20 pou, Dukes C phase 20 pou, Dukes D phase 20 pou colorectal cancer paraffin masses. Results In colorectal carcinoma tissues, anisoclinical phase colorectal carcinoma lymphatic microvessel density were: Dukes A phase(1.37±0.25)pieces/mm^2, Dukes B phase (1.49±0.31) pieces/mm^2, Dukes C phase (2.61±0.58)pieces/mm^2 , Dukes D phase (2.87±0. 57) pieces/mm^2. There was a significant difference between them (P〈0.05); maximal profile average area were : Dukes A phase .(9.67±1. 14)μm^2, Dukes B phase (10.62±2. 22)μm^2, Dukes C phase (18.46±4. 15)μm^2, Dukes D phase (20. 30±4. 07)μm^2, there was a significant difference between them too (P〈0.05). In no metastasis grope (DukeA and DukeB)lymphatic microvessel density was (1.43 ± 0.28) pieces/mm^2 , maximal profile average area was ( 10.15 ± 1.41)μm^2 ;in lymph node metastasis grope(Duke C and Duke D) lymphatic microvessel density was (2. 74±0. 58)pieces/mm^2 , maximal profile average area was (19.38±4.16)μm^2 ; there were significant difference between them(P〈0.01). Conclusion There exists lymphangiogenesis in the colorectal carcinoma peritumoral tissues, and from high differentiation to poor differentiation; In colorectal carcinoma, clinical phase later maximal profile average area increase gradually, it may say that lymphangiogenesis is an important role in metastasis in colorectal carcinoma.
