不同照射野术中放疗对肝门部胆管影响的实验研究

摘要 [目的]探讨不同照射野的术中放疗对肝门部胆管的影响。[方法]12只小猪随机分成3组(n=4)。所有动物均接受肌囊切除和肝外胆管部分切除。此外,Ⅰ组动物接受20Gy的肝门部术中放疗,范围是直径5 cm的圆形照射野。Ⅱ组动物亦接受20 Gy的肝门部术中放疗,而范围是直径7 cm的圆形照射野。Ⅲ组为实验对照组,不进行术中放疗。[结果]Ⅰ、Ⅱ组动物死亡率无统计学上的差异(P=0.49)。术后定期PET和CT影像显示:放疗后第14天放疗区肝组织对FDG的摄取量明显减弱(P<0.05),而第56天时则恢复正常。尸检及病理组织学检查显示:放疗动物的胆管空肠吻合口均出现狭窄和纤维化改变,肝内胆管扩张和胆汁淤积伴化脓性胆管炎。[结论]5 cm照射野与7 cm照射野术中放疗对肝门部胆管影响无显著差异。
