戊型肝炎病毒感染与胰腺癌发生的关系

Relationship between pancreatic cancer and infection of hepatitis E virus

目的探讨戊型肝炎病毒(HEV)感染与胰腺癌发生的关系。方法以住院患者为研究对象,包括2900例病毒感染者(9例甲型肝炎、2129例乙型肝炎、81例丙型肝炎、146例戊型肝炎和535例EB病毒感染)和5569例对照,检测患者血清甲、乙、丙、戊型肝炎病毒标志物和EB病毒EB-VCA-IgA,收集相关临床资料包括CD4/CD8比值、总胆红素、糖类抗原(CA19-9)、γ-谷氨酰转肽酶(γ-GT)和脂肪酶等,比较HEV和其他病毒感染与胰腺癌的关系。结果在HAV、HBV、HCV、HEV、EBV感染组中胰腺癌病例分别为0、5、1、12、1例,与对照组相比,HEV感染组罹患胰腺癌的风险增加约48倍(OR49.16,CI1.31～41.23),而其他病毒感染组和对照组之间差异无统计学意义(P>0.05),而且HEV感染的胰腺癌患者血清CA19-9、总胆红素和γ-GT均增高,而CD4/CD8降低。结论戊型肝炎病毒感染可能与胰腺癌的发生有一定的关系,HAV、HBV、HCV、EBV病毒感染与胰腺癌发生无关。

Objective To investigte the relationship between the infection of hepatitis E virus and pancreatic cancer. Methods The patients included 2 900 casess of viral infection （9 cases of HAV,2 129 cases of HBV, 81 cases of HCV, 146 cases of HEV and 535 cases of EBV）, 5569 cases of non viral infection as normal control. Adop ring case control study, the antibodies of HAV, HBV, HCV, HEV and EBV were tested, the related clinical data were also collected, and the results were compared. Results There were 12 cases of pancreatic cancer in HEV infection, 9 in HBV, 1 in HCV, 1 in EBV. Compared with the normal control, the risk to pancreatic cancer in HEV group increased 48 times（OR 49.16, CI 1.31-41.23）. There was no significant difference between other viral infection groups and the control group. Conclusion HEV infection may have some relation to occurrence of pancreatic cancer.
