摘要
1995年1月~1996年12月我院应用空心钉治疗股骨颈骨折27例。有以下优点:1.空心钉及手术器械均为中空结构,损伤小,操作简便,定位准确。2.三角形排列,稳定性好,骨折端加压可靠,术后X线片骨折线模糊不清。3.固定角度140°左右,符合股骨颈骨小梁拱形排列顺序,使骨折剪切力变成较大轴向力,符合生物力学要求。三角形排列,牢固固定加该钉头部螺纹对固定后股骨头的自锁能力,促进了骨折愈合,明显降低了股骨头坏死及骨不愈合发生率。4.本手术固定方法抗弯、抗扭、抗剪力强。手术适应症是股骨颈骨折GardenⅡ~Ⅳ型及粗隆间骨折Ⅰ型。术后可早期、无痛、主动活动髋关节,骨折愈合快,明显降低了骨折并发症的发生。
Summing up the application of hollow nail treatment of 27 patients with femoral neck fracture treated in our hospital between 1995 and 1996,we found the following advantages:It is simple and convenient with accurate localization and least injury by the hollow structure of nails and other appliances.The triangular arrangement makes the satisfying stability and reliable pressure on fracture ends(Dim image,in X-ray).The fixed angle(≈140)is identical with the arch arrangement of bony trabecula, then the more shear force changes into axial one. By adding the thread head of nails, the self-lock ability of fixed caput femoris is strengthened, and knitting time is shortened. The method has great resistance to bending, flexing and shearing force. Indication: femoral neck fracture(GardenⅡ~Ⅵ),intertrochanteric fracture(type I). Hip joint exercise can be done earlier after the operation with out pain and knitting is quick. The complications of fracture can be controlled to the lowest limit.
