口服降糖药联合甘精胰岛素治疗2型糖尿病的临床观察

Clinical Observation of Oral Hypoglycemic Drug Combined with Glargine in the Treatment of Type 2 Diabetes Mellitus

目的观察甘精胰岛素联合口服降糖药治疗2型糖尿病(T2DM)的临床效果。方法对56例单用口服降糖药效果欠佳的T2DM患者早餐前加用甘精胰岛素治疗,分别于治疗前及治疗后观察空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbAlc)、空腹C肽(FCP)、餐后2hC肽(2hCP)、血脂、体重、血压的变化.结果早餐前加用甘精胰岛素治疗后FPG、2hPG、HbAlc较治疗前明显下降(P<0.01),治疗后3个月FCP、2 hCP较治疗前明显升高(P<0.01),而对血脂、体重指数、血压影响不大,且无低血糖反应。结论口服降糖药物联合甘精胰岛素治疗方案具有降糖作用佳、安全性良好的特点。

Objoctive To evaluate the clinical effects of glargine combined with oral hypoglycemic drug in the treatment of type 2 diabetes. Mothods Twenty - five type 2 diabetic patients with inadequate glycemia control were given glargine before breakfast. The levels of fasting plasma glucose and C - peptide （ FPG and FCP）, 2 hours postprandial glucose and C -peptide （2 hPG and 2 hCP）, HbA1 c, lipids （TC, TG）, weight and blood pressure were compared before and after treatment. Rsults After the treatment, FPG （ 14vs7 mmol/L）, 2 hPG （18vs9mmol/L） and HbAlc （ 11%vs7% ） significantly decreased （P 〈0. 01 ）. FCP and 2hCP significantly increased before and after 3 months （P 〈0. 01 ）. There was no difference in lipids, body mass index （BMI） and blood pressure. There was no hypoglycemia. Conclusion Excellent glycemia control and satisfactory safety can be induced by oral hypoglycemic drug with glargine therapy in type 2 diabetic patients.
