河南省村级医疗机构法定传染病漏报原因分析

ANALYSIS ON THE REASONS FOR MISSING REPORT OF NOTIFIABLE INFECTIOUS DISEASES IN RURAL MEDICAL UNITS IN HENAN PROVICE

[目的]找出河南省农村法定传染病漏报严重的原因,为制定相关政策提供依据。[方法]采用分阶段整群抽样法,从河南省报告水平中等县随机抽出一个平原县和一个山区县作为样本县,每个样本县随机抽4个乡镇作为样本乡,对样本乡所有村级医疗机构的法定传染病责任报告人进行调查,统计分析采用非条件logistic分析。[结果]"有乡村医生执业资格","是防保医生","防保工作与乡卫生院有合同协议"均是村医报告传染病的促进因素。[结论]没有乡村医生执业资格、非防保医缺乏报告意识以及防保医生与乡卫生院没有合同协议均是我省村级医疗机构传染病报告漏报严重的政策方面的原因。

[Objective]To find out the reasons of serious missing report of notifiable infectious diseases in rural areas in Henan Province,and to provide evidence for making relevant policy.[Methods]Adopting multistage stratified cluster sampling,a plain county and a mountain county were chosen randomly as samples from those countries whose report level of notifiable infectious diseases was secondary in Henan Province,4 towns were sampled as sample towns randomly from each sample county,then all the responsible reporters in rural medical units for notifiable infectious diseases in 8 sample towns were investigated.The results were analyzed using non-conditional Logistic regression analysis.[Results]Promoting factors for reporting infectious diseases for rural doctor were having the qualification as a rural doctor,being a preventive doctor and having contract with rural hospital on anti-epidemic work.[Conclusions]The main reasons that affects notifiable infectious diseases reporting in rural medical units in Henan Province are lacking of consciousness in reporting notifiable infectious diseases resulted from without qualification as a rural doctor,and not being preventive doctors,as well as having no contract signed with rural hospital on preventive work.
