烧伤后体表及器官瘢痕畸形的修复整形

Clinical Report of Plastic Recovery of Deformed Scar of the Organs on the Body Surface.

目的:探讨体表器官瘢痕畸形的外科治疗,使其外观最大限度恢复原形。方法:①瘢痕切除"Z"字或"W"成形;②瘢痕切除全、中厚皮片移植修复;③瘢痕切除局部皮瓣移植;④瘢痕切除远位皮瓣或扩张后皮瓣移植。结果:治疗3568例,效果优良2378例,优良率66.65%,效果良好1184例,良好率33.18%,效果差6例,较差率0.17%;在治疗随访半年以上的病历中,局部逐渐恢复正常,外观也较稳定。结论:体表器官瘢痕畸形的修复,虽然采用不同厚度的皮片,皮瓣移植和"Z"字成形等,均为传统治疗修复,但只要依据瘢痕程度及形状,选择恰当的修复方法,治疗均能达到理想的效果。

Objective： To approach surgical treatment of deformed scar of organs on the body surface so as to make them recovered to their original status maximatily. Method： 1 ） The scar was excised into ＂Z＂ or ＂W＂ ; 2） Having scar excised, whole or intermediate thickness split skin grafting was performed for recovery; 3） Having scar excised, regional flap grafting was performed; 4） Having scar excised, distant skin flap grafting or flap grafting after expanding was per- formed. Results： Total 3568 cases were treated. Of them, 2378 cases appeared excellent or better with rate 66. 65% ; 1184 cases appeared good with rate of 33. 18% ; only 6 cases appeared poor or bad with rate of 0. 17%. In the records of following visit of more than a half of year, operation region was recovered into normal and appeared in stable status. Con- clusion： Although various skin grafting techniques, such as different thickness split or flap grafting and ＂Z＂ forming et al, which are all conventional ways, idea effect can be obtained as long as correct measures were taken on the basis of scar status.
