剥脱综合征手术治疗临床分析

Clinical analysis of operative treatment for exfoliation syndrome.

目的观察分析剥脱综合征发病原因,探讨其合并白内障、青光眼手术治疗的效果、并发症及注意事项。方法临床病例16例(18眼)。全部合并白内障。行晶状体超声乳化囊袋内人工晶状体植入术。其中同时合并青光眼7例(7眼),行晶状体超声乳化、囊袋内人工晶状体植入、小梁切除三联术。结果剥脱综合征早期多无自觉症状,合并白内障、青光眼后,晶状体悬韧带松脆易断,手术时容易发生悬韧带断裂,玻璃体脱出等并发症。16眼手术顺利,占88.89%,2例出现并发症,占11.11%。结论剥脱综合征合并白内障、青光眼应及早手术。行品状体超声乳化术囊袋内人工晶状体植入术及小梁切除术是较安全且效果良好。但对并发症的发生要格外注意和妥善处理。

Objective Analyzing the mechanism of ES and operative effect,complications and important points when ES com- plicated with cataract,glaucoma. Methods 16 （18 eyes） patients with cataract and were treated through phacoemulsification and IOL implantation,7 patients （7 eyes） also complicated with glaucoma were treated through phacoemulsification,IOL implantation and tra- beculectomy. Results In early stage, ES patients had few symp- toms,when it complicated with cataract, glaucoma, the suspending liament of lens became fragile. In operation, 16 eyes （89%） were operated successfully,2 eyes （ 11% ） had complications. Conclu- sions ES complicated with cataract,glaucoma should be operated early. The phacoemulsification, IOL implantation and trabeculecto- my is safe and effective operative method, but complications should be paid more attention to avoid and dealt with carefully.
