经皮内镜下胃/空肠造口术在家庭肠内营养支持患者的应用

Percutaneous endoscopic gastrostomy/jejunostomy: An effective method for home enteral nutrition support

目的:研究经皮内镜下胃/空肠造口术(PEG/J)在家庭肠内营养(HEN)患者应用的可行性、疗效和并发症。方法:36例患者接受PEG/J行HEN,观察置管的时间、成功率、并发症及疗效。结果:PEG操作时间平均(6.4±2.1)min,PEJ平均(15.7±4.2)min。技术操作成功率为100%,置管操作时未发生并发症,置管后并发症发生率为5.6%(2/36)。患者在接受HEN2个月后,体质量、体质指数(BMI)、血浆蛋白较支持前均明显提高,而Karnof-sky行为状态分级评分较支持前也有显著改善。结论:经PEG/J行HEN操作简便、安全,可以显著改善患者的营养状况,提高患者的生活质量。

Objective：To investigate the use of percutaneous endoscopic gastrostomy / jejunostomy （PEG/J） in patients on home enteral nutrition（HEN）. Methods：From January 2004 to March 2007, 36 patients received percutaneous endoscopic gastrostomy / jejunostomy for HEN, including 21 males and 15 females. The duration, success rate, complications and effectiveness of this procedure were analyzed. Results ：The tubes were successfully placed in 100% of the cases within an average of （6.4 ± 2.1 ） min in PEG and （15.7 ± 4.2） min in PEJ. No procedure-related complications were observed except some minor ones in 2 cases （5.6%）. Significant improvement was noted in the weight, body mass index, plasma protein and quality of life of the patients after 2 month HEN support. Conclusion ： Gastrostomy / jejunostomy via the percutaneous endoscopic route, easy to perform and safe in use, are effective methods HEN support.
