200例卒中后癫痫的临床研究

A clinical study of postapolectic epilepsy

目的探讨脑卒中后癫痫的临床特点及发病机制。方法对2 500例脑卒中病例中200例继发癫痫的临床资料进行回顾性分析。结果卒中癫痫的发生率为8.00%,早期癫痫发作为4.30%,晚发性癫痫为3.90%,卒中后癫痫的发生率与病灶部位(皮层/或皮层下)差异有统计学意义(P<0.01),与卒中类型差异无统计学意义(P>0.05)。结论脑水肿是早期癫痫的发作最重要的致病因素,晚期发作癫痫是因胶质疤痕等因素形成癫痫灶所致,建议对晚期发作癫痫进行严格、系统而有效的治疔。

Objective To probe the clinical features and the onset mechanism of postapolectic epilepsy, Methods The clinical data of 200 secondary epilepsy taken from 2 500 cerebral apoplexies were studied retrospectively. Results The incidence of the postapolectic epilepsy 8.00% ,the epileptic attack in early stage was 4.30% and that in late stage was 3.90%. The incidence of the postapolectic epilepsy was quite different accoding to the location of focus（cortex/subcortex）. Conclusion The cerebral edema is an important factor of the epileptic attack in early stage while thw late epilepsy may be caused by the epileptic focus resulted from the neuroglial scar. It is sugested that the strict, systematic and effctive therapy should be taken in the late stage of epilepsy.
