骨髓切片组织病理检查在血液病诊断中的临床应用

Clinical application of bone marrow biopsy on diagnosis for hematopathy

目的研究骨髓切片组织病理检查对血液病的诊断价值。方法采用骨髓活组织病理学、骨髓涂片细胞形态学检测方法。结果300例血液病患者中骨髓细胞形态学检查与骨髓活检结果完全吻合者215例(占71.67%),骨髓形态学不能肯定诊断而经骨髓活检结果确诊的病例30例(占10%),骨髓细胞形态学检查与骨髓活检结果不相吻合的有55例(占18.33%)。结论骨髓活检病理诊断是对骨髓形态学检查的补充和修正,二者合用能提高血液病诊断的正确率。在MDS、AA、MF的诊断上,骨髓活检病理检查具有决定性的意义,对转移性肿瘤和急性髓细胞白血病是否合并骨髓纤维化的诊断也具有特殊价值。

Objective To explore the clinical effect of bone marrow biopsy on diagnosis for hematopathy. Methods To diagnose hematopathy with bone marrow biopsy and bone marrow aspiration. Results In 300 hemopathic patients, 215 （71.67%） patients＇ final diagnosis by bone marrow biopsy were coincide with bone marrow aspiration. Thirty （10%） patients which could not make a definite diagnosis by bone marrow aspiration were diagnosed by bone marrow biopsy. Fifty-five （18.33 %） patients＇ final diagnosis by bone marrow biopsy were not coincide with bone marrow aspiration, Conclusion Bone marrow biopsy and aspiration are complementary techniques for diagnosis and evaluation of patients with hematologic disease, especially with myelodysplastic syndrome, myelofibrosis, aplastic anemia and metastatic carcinoma and acute myelocytic leukemia togethor with myelofibrosis.
