超声在喉癌颈部淋巴结转移中的诊断价值

Diagnostic Value of Ultrasonography in Metastases of Cervical Lymph Nodes in Patients with Larynx Carcinoma

目的:探讨超声在喉癌颈部淋巴结转移中的诊断价值。方法:应用超声检查53例术前喉癌患者的颈部淋巴结,所见与手术病理结果对照分析。结果:超声检查发现并与手术切除对照符合的淋巴结总数196个,超声诊断淋巴结转移的敏感性为92.0%,特异性为86.9%,准确性为89.8%。多因素Logistic回归分析显示,淋巴结内部回声不均、髓质变形或缺失、周边型及混合型血流预示淋巴结转移。结论:超声在喉癌颈部淋巴结转移的诊断中具有较高的应用价值。

Objective To investigate the value of ultrasonography in diagnosis of the cervical lymph nodes （CLN） metastases of larynx carcinoma. Methods CLN of 53 patients with larynx carcinoma were observed before surgery, the results were compared with surgery and pathological findings. Results A total of 196 CLN in 53 cases were analyzed. The sensitivity, specificity and accuracy of ultrasonography in evaluating metastasis were 92.0 % ,86.9% and 89.8 %. Multivariate logistic regression analysis revealed that lymph nodes with inhomogenous internal echo, eccentric medulla or without medulla, Ⅲ and Ⅳ vascular pattern suggested metastasis positive. Conclusion Ultrasonography is valuable in detecting metastatic lymph nodes in patients with larynx carcinoma.
