布托啡诺对丙泊酚靶控输注患者镇静效应的影响 被引量:5

摘要 目的研究阿片类受体激动-拮抗剂布托啡诺对靶控输注(TCI)丙泊酚患者镇静效应的影响。方法选择择期全麻手术患者40例(ASAⅠ~Ⅱ级),年龄18~59岁,随机双盲分为两组,布托啡诺组(B组,n=20)和对照组(C组,n=20)。全麻诱导前启动丙泊酚TCI,血浆靶浓度1.5μg/ml,5min后B组静脉注射布托啡诺50μg/kg(30s),C组静脉注射生理盐水5ml。记录丙泊酚TCI启动前(T0),丙泊酚TCI启动后1min(T1),3min(T2),5min(T3),分组给药后1min(T4),3min(T5),5min(T6)各时点BIS,AAI,OAA/S评分,及其MAP、HR和SpO2。结果靶控丙泊酚1.5μg/ml后1~5min,两组患者BIS,AAI值及OAA/S镇静评分均显著下降,其中T1~3与T0相比差异具有显著性(P<0.05或P<0.01);B组静脉注射布托啡诺50μg/kg后,BIS和AAI分别再降低11.2%~30.3%和20.9%~44.2%,与T3比较,差异具有显著性(P<0.05或P<0.01),BIS和AAI值分别降低达54±7和24±6,其相应OAA/S评分也显著降低,MAP轻度降低(P<0.05);两组HR和SpO2均在正常范围内波动(P>0.05)。结论布托啡诺与丙泊酚联合用药具有协同作用,静脉注射布托啡诺50μg/kg可加深血浆浓度1.5μg/ml丙泊酚靶控的镇静程度,患者由清醒镇静状态进入睡眠状态,可降低BIS和AAI值。
