阴道B超与宫腔细胞学检查在子宫内膜癌筛查中的应用

Application of transvaginal color doppler sonography combined with endometrial sampling device for cytology in screening of endometrial cancer

目的探讨阴道B超、宫腔吸片细胞学检查在子宫内膜癌筛查中的应用价值。方法采用前瞻性队列研究对同一人群进行为期2年的两轮随访检查,对有子宫内膜病变可疑症状的人群进行阴道B超测量子宫内膜厚度,内膜厚者进行宫腔吸片细胞学检查,异常者进行诊断性刮宫。结果第1次检查发现子宫内膜癌和不典型增生各1例,复杂增生3例,2年后的第2次筛查发现复杂增生1例。结论阴道B超、宫腔吸片细胞学联合检查在子宫内膜癌筛查中有一定的使用价值。

Objective To probe into the value of transvaginal color Doppler Songraphy（TVCDS） combined with endometrial sampling device for cytology（ESDC） in screening endometrial cancer.Methods 567 women（a prospective cohort） were examined two times during two years follow-up.The women who had a higher risk of endometrial carcinoma or precancerous pathological changes were examined by TVCDS.If endometrial thickness≥5 mm for post-menopausal women or endometrial thickness≥10 mm for the reproductive women,the women were further examined by ESDC.If the result of ESDC showed to be suspicious,fractional curettage was performed to get the final diagnosis.Results One case of endometrial cancer,one case of atypical hyperplasia and three cases of simple hyperplasia were diagnosed during the first examination,and one case of complex hyperplasia was diagnosed in the second examination two years later.Conclusion TVCDS combined with ESDC was helpful in endometrial cancer screening.
