摘要
目的从IgA分子糖基化机制的角度研究黄芪对IgA肾病(IgAN)的治疗作用。方法取IgAN患者和健康对照者外周血淋巴细胞随机分为空白、植物血凝素(PHA)、PHA+黄芪3组,培养5天,培养前后进行细胞计数并留取上清液作ELISA分析,测定IgA浓度并以IgA1与蛋白凝集素的结合力分析其糖基化程度。结果IgAN患者和正常人外周血淋巴细胞数无明显差异(P>0.05),经干预后外周血淋巴细胞增生率均有上升,且PHA+黄芪共刺激作用后外周血淋巴细胞增生率[IgAN组(10.16±0.74),对照组(10.97±1.43)]明显高于单用PHA刺激后的各组[IgAN组(8.72±0.74),对照组(8.68±1.20),P<0.05];在未加任何干预的情况下,IgAN组细胞培养上清液内IgA浓度明显高于对照组(P<0.05),在PHA刺激后两组细胞培养上清液内IgA浓度均有明显上升(P<0.05),加用黄芪共刺激后IgA浓度与单用PHA刺激组相比无明显变化(P>0.05)。无干预情况下IgAN组与对照组相比较,IgA1分子与蚕豆凝集素的结合力明显增高(P<0.05),在加用黄芪干预之后两者结合显著受抑(P<0.05)。结论黄芪可促进PHA刺激下正常人和IgAN患者外周血淋巴细胞的增生,IgAN患者体内淋巴细胞分泌IgA能力明显高于正常人,且IgA1分子呈低糖基化,黄芪体外对IgA1糖基化异常有明显改善作用。
Objective to investigate the effect ofAstragalus mongholicus on the galactosylation of IgA1 in Immunoglobulin A nephropathy(IgAN) patients. Methods 20 mL venous blood was obtained from 15 patients with biopsy-proven IgAN, and 10 normal individuals. The lymphocytes were isolated and cultured in vitro. The cells cultured were divided into control group, PHA -stimulated group and PHA-Astragalus mongholicus co-stimulated group. The IgA concentration and galactosylation degree in the supernatant was tested by ELISA after 5 days and the cells were counted before and after culturing. Results There was no apparent difference in the number of lymphocytes between IgAN patients and normal control(P〉0.05). The lymphocytes had more significant proliferation after stimulated by PHA and Astragalus mongholicus than only by PHA (P〈0.05). The IgA levels in the IgAN patients were remarkable higher than those in the normal control. Astragalus mongholicus didn't show the facilitative effect on IgA secretion of lymphocytes. The binding of IgA1 with lectin was remarkably higher in IgAN patients than those in the normal control. After cultured with Astragalus mongholicus, the binding decreased obviously in IgAN patients(P〈0.05). Conclusions Astragalus mongholicus didn't show the facilitative effect on IgA secretion of lymphocytes. IgA1 in IgAN patients were under-glycosylated and Astragalus mongholicus can rebuild the normal galacosylation of IgA1 in IgAN patients.
