健择联合卡铂治疗中晚期非小细胞肺癌的临床研究

Clinical observation on efficacy of gemcitabine combined with carboplatin in treating the senile patients with advanced non-small cell lung cancer

目的观察健择为主联合化疗方案治疗老年中晚期非小细胞肺癌的临床疗效及毒副反应。方法50例老年中晚期非小细胞肺癌患者随机分为两组。健择单药组30例,采用健择1200 mg/m2静滴,第1、8天。联合化疗组20例,采用健择1200 mg/m2静滴,第1、8天;卡铂300 mg/m2,静滴,第2天。完成3个周期以上治疗的患者按WHO标准进行临床疗效及毒副反应评估。结果单药组有效率为26.7%(8/30),联合化疗组为55.0%(11/20),两组比较差异有显著性(P<0.05);联合化疗组毒副反应发生率较单药组高(P<0.01),主要毒副反应为骨髓抑制。结论健择单药应用毒副反应轻,能有效提高老年患者生存质量及用药安全性。

Objective To investigate the efficacy and side effects of combined chemotherapy based on gemcitabine in treatment of the aged with advanced non-small-cell lung cancer（NSCLC）. Methods Fifty patients in this study were divided into monotherapy group（ 30 patients）and combined therapy group （20 patients）. Those in monotherapy group were given gemcitabine 1200 mg/m^2on the first day and eighth clay,and those in combined therapy group received gemcitabine 1200 mg/m^2 on the first day and eighth day and carboplatin 300 mg/m^2on the second day. All the drugs were given intravenously. The efficacy and adverse effects were evaluated according to WHO Standard after 3 cycles of treatment. Results The response rate of montherapy group was 26.7% （8/30） and that of the combined therapy group was 55.0% （11/20） ,the difference was significant （P 〈 0.05 ）. The incidence of adverse effects was higher in the combined therapy group （P 〈 0. 01 ）. The major toxic reaction was bone marrow depression. Conclusion The toxic reactions of monotherapy with gemcitabine is mild, which improves the safety of medication and the quality of life in aged patients.
