颞下颌关节疾病的关节内窥镜术

Arthroscopy of the Temporomandibular Joint Disorders

收集了４５例颞下颌关节内紊乱与骨关节病的病例，在结合临床和Ｘ线检查的基础上，利用关节内窥镜观察关节腔内的病理改变并同时进行关节腔内生理盐水冲洗，纤维粘连剥离，关节盘的手法复位等外科治疗术。结果表明：关节内窥镜对颞下颌关节内紊乱和关节盘移位、骨膜炎症、纤维粘连、关节盘穿孔以及骨关节病的关节结构退行性变均有特殊的诊断价值。关节内窥镜的外科治疗其近期疗效是肯定的，但还需进行长期的观察。

In order to investigate the arthropathy of intracapsular conditions, an arthroscopic examination was performed on 45 joints with diagnosed internal derangement and osteoarthrosis of the temporomandibular joint. After diagnostic arthroscopy was completed, arthroscopic surgery was made by lavage, lysis, with or without disc manipulation. We conclude that TMJ arthroscopy is very important in detecting and localizing disc displacement, synovitis, articular degenerative change, adhesions and perforations. The results of surgical arthroscopy in short term is good and a long term follow up will be necessary.
