单纯性肥胖者白天和夜间自主神经变化特征

摘要 目的观察单纯性肥胖者白天和夜间自主神经的活动特征。方法采用美国PI(Preideal Instruments)公司生产的十二导联动态心电记录仪,对102例单纯性肥胖者和49例正常体重健康志愿者,记录24h心电信号,进行白天和夜间心率变异分析。结果①白天肥胖组RR间期标准差(SDNN)、RR间期差值的均方根(RMSSD)、低频功率(LF)、高频功率(HF)均显著高于对照组;LF/HF肥胖组显著低于对照组,p<0.01。②夜间肥胖组SDNN、RMSSD、LF、HF显著高于对照组,p<0.01;LF/HF两组之间无差异,p>0.05,③对照组LF/HF夜间显著低于白日,p<0.05;肥胖组LF/HF夜间与白日无显著差异,p>0.05。结论肥胖者无论是白日还是夜间,迷走神经功能始终处于亢进状态。迷走神经活性增高,可能是导致胃动力亢进,肥胖形成的主要原因之一。 Objective To explore the characteristics of autonomic activity on day time and at night in obese people. Methods 102 obese volunteers and 49 normal controls were studied with 24 hour dynamic electrocardiography (PI)and HRV analysis was performed. Results (1)on day time , SDNN, RMSSD, LF, HF in the obese were higher than the control's, LF/HF ratio in the obese was lower than the control's,p〈0.01. (2)at night, SDNN, RMSSD,LF, HF in the obese were higher than the control's, p〈0. 01 ;LF/HF ratio in the obese was similar to control's, p〉0.05. (3) in the control,LF/HF ratio on day time was lower than at night, p〈0.05;in the obese, LF/HF ratio on day time was similar to at night,p〉0.05. Conclusion obese people have hyperactivity of vagal function whether on day time or at night. Vagal activity increased may be one mechanism of hypermotility and obesity genesis.
