选择性ERCP在腹腔镜胆囊切除术中的应用

THE APPLICATION OF SELECTIVE ERCP IN LAPAROSCOPIC CHOLECYSTECTOMY

目的探讨术前选择性内窥镜逆行胰胆管造影术(endoscopic retrograde cholangiopancreatography,ERCP)检查在腹腔镜胆囊切除术中对预防胆道损伤的作用。方法比较开展术前选择性ERCP检查前后胆管损伤的发生情况。结果2003年开展术前选择性ERCP前行腹腔镜胆囊切除术326例,胆道损伤4例;开展后行腹腔镜胆囊切除术783例,未发生胆道损伤。结论术前选择性ERCP能明显降低胆道损伤的危险。

Objective To assess the value of preoperative selective endoscopic retrograde cholangiopancreatography （ERCP） in the preventing of biliary tract impairment during laparoscopic cholecystectomy（LC）. Methods A retrospective review of biliary tract impairment in LC with preoperative selective ERCP and that without was conducted. Results There were 4 cases of biliary tract impairment among 326 cases of LC without preoperative selective ERCP; while there was no biliary tract impairment among 783 cases of LC with preoperative selective ERCP since 2003. Conclusion Preoperative selective ERCP can decrease the risk of biliary tract impairment during laparoscopic cholecystectomy.
