双钢板内固定在复杂型肱骨髁间骨折中的应用

Dual Plating Internal Fixation for the Complicated Humeral Intercondylar Fracture

目的探讨双钢板内固定治疗复杂型肱骨髁间骨折的手术方法和临床疗效。方法回顾性分析采用双钢板内固定治疗复杂型肱骨髁间骨折25例，术后随访时对骨折复位质量、异位骨化、X线表现、肘关节功能分别进行评估。结果25例患者均获11～29个月、平均19．2个月随访，术后1个月，肘关节伸屈活动为0°～100°，术后12个月肘关节伸屈活动为0°～135°。骨折愈合时间12～20周，平均14．5周。按Jupiter肘关节评分系统评价肘关节功能；优9例，良12例，尚可4例，优良率84％。结论双钢板内固定能提供牢固持久的固定，显著降低相关并发症，促进肘关节功能的恢复，是治疗复杂型肱骨髁间骨折的理想方法。

Objective To investigate the treatment of humeral intercondylar fracture by dual plating internal fixation. Methods 25 cases of humeral intercondylar fracture were treated with approach of osteotomy of oiecranon and dual plating internal fixation. Postoperative reduction quality, radiographic results, heterotopic ossificationand function of elbow were evaluated separately. Results 25 cases were followed up for 11 months to 29 months （average 19.2 months）. According to the criteria of Jupiter, the function of elbow was excellent in 9 cases, good in 12 cases, fair in 4 cases, The excellent and good rate was 84%. Conclusion Dual plating internal fixation can provide rigid internal fixation, and decrease the related complications significantly. It can be used as an effective method in the treatment of the humeral intercondylar fracture.
