摘要
目的研究颅脑外伤患者脑脊液及血液中IL-6的表达,探讨IL-6水平与颅脑外伤的病情程度和预后是否有关。方法采用双抗体夹心酶联免疫吸附法(ELISA)。测定36例颅脑外伤患者脑脊液和血清IL-6含量。患者分为两组,A组(轻中型颅脑损伤组)16例,GCS〉8分;B组(重型颅脑损伤组)20例,GCS≤〈8分。结果IL-6在脑脊液和血液中浓度,第3天达到高峰,第7天开始下降。脑脊液中IL-6水平与颅脑外伤的严重程度呈正相关,损伤程度重者IL-6含量明显增高,且持续时间较长。血清IL-6水平与临床病情和炎症反应变化呈平行关系,脑脊液中IL-6在伤后明显高于血清中IL-6值。结论颅脑损伤患者脑脊液及血清存在1L-6的过量表达,且脑脊液表达较明显。IL-6的检测可作为反映脑损伤严重程度及预后转归的指标。
Objective In this study, the expression of IL-6 in serum and CSF was detected in patients with craniocerehral injury. The aim of the study is to explore the correlation of IL-6 level and the severity as well as prognosis of craniocerebral injury, so as to provide a guide to clinical therapy, illness state judgement and prognosis evaluation. Methods According to GCS, 36 patients were divided into two groups, mild medium craniocerebral injury group (n = 16, GCS〉8 score) and severe craniocerehral injury group (n= 20,GCS≤8 score). Double antibody sandwich enzyme linked immunosorhent assay (ELISA) was adopted to determine IL-6 level in cerebrospinal fluid (CSF) and serum from 36 patients with craniocerebral injury. Results There was significant difference of IL-6 level between the two groups. The peak value of IL-6 in CSF and serum emerged on day 3 after injury, and began to decline on day 7. IL-6 concentration in CSF was positively correlative to severity of craniocerehral injury. The more severe the injury, the higher the IL-6 concentration and the elevated IL-6 level lasted for a long time, indicating that IL-6 mostly came'from central nervous system during brain injury. The variation of serum IL-6 level paralleled with the state of illness and inflammatory reaction. After injury, IL-6 level in CSF was much higher than that in serum. Conclusion Over expression of IL-6 in serum and CSF is present after brain injury especially in CSF, which revealed that the determination of IL-6 in CSF is more valuable than in serum. IL-6 may he used as a sensitive marker not only for the judgment of injury severity and inflammation reaction but also for the diagnosis, treatment surveillance and prognosis evaluation.
