心电图对急性下壁心肌梗死罪犯血管的预测价值 被引量:1

摘要 目的探讨急性下壁心肌梗死(AMI)时体表心电图改变与罪犯血管的关系。方法回顾性分析56例急性下壁心肌梗死患者心电图改变与冠状动脉造影结果。结果(1)STⅡ/Ⅲ比值<1预测右冠状动脉(RCA)为罪犯血管的敏感性为90%,特异性为93%。(2)STⅡ/Ⅲ比值=1预测罪犯血管为左回旋支(LCX)的敏感性为88%,特异性为90%。(3)I导联ST段位于等电位线预测病变血管为LCX的敏感性为87%,特异性为76%。(4)I导联ST段压低预测RCA病变的敏感性为76%,特异性为93%。(5)STⅡ/Ⅲ=1并V6导联ST抬高预测LCX病变的敏感性为40%,特异性为100%。(6)STⅡ/Ⅲ<1并V6导联ST段下降预测RCA病变的敏感性为29%,特异性为100%。结论STⅡ/Ⅲ≥1,I导联ST段位于等电位线及V6导联ST段抬高有助于预测LCX病变;而STⅡ/Ⅲ<1,I导联ST段压低及V6导联ST断压低有助于预测RCA病变。
