心率震荡在评估急性心肌梗死患者预后中的价值研究 被引量:2

摘要 目的研究新的心电学指标——心率震荡(HRT)在急性心肌梗死患者中对近期预后的预测价值。并与传统指标——心率变异(HRV)进行比较,分析心率震荡与心率变异的相关性。方法33名急性心肌梗死后窦性心律的患者入选,平均随访6个月。心率震荡的值用两个参数表示TO(震荡起始)、TS(震荡斜率)。TO、TS通过24小时动态心电图计算获得。同时利用动态心电图对患者心率变异的时域指标SDNN进行测量。终点事件是心源性死亡。结果心肌梗死后死亡组(5例)TS明显减弱(P<0.05),TO明显升高,与幸存组(28例)比较差异均有显著性(P<0.05)。TS与SDNN之间存在相关性(r=0.447,P=0.009)。结论HRT参数TO和TS值在急性心肌梗死患者预后的评估中有重要价值。
