120例首发精神分裂症患者5年随访调查研究

Five year follow-up study of one hundred and twenty first onset schizophrenia

目的:探讨首发精神分裂症患者远期预后及其影响因素。方法:运用标准化评定工具对120例首发精神分裂症患者进行5年随访调查及预后评定,对预后有关影响因素作单因素分析和Logest回归因素分析。结果:首发精神分裂症患者5年随访调查时总体结局良好21例(17.5%),一般34例(29.2%),不良64例(53.3%)。单因素分析有11项观察指标与预后有关,多因素分析显示远期预后主要影响因素依次为病前职业功能水平、治疗依从性、起病形式和家庭关系。结论:首发精神分裂症患者远期预后受生物、心理、社会多种因素影响,总体结局较差,其中部分影响因素可进行干预。

Objective： To study the factors that might affect the prognosis of the first onset schizophrenia. Methods： One hundred and twenty first onset schizophrenic patients were enrolled and followed up 5 years. All the data were collected and conducted the single - factor- analysis and Logest analysis. Results： The result showed that 21 patients（17.5% ） were markedly improved,35（29.2% ）improved,and 64（53.3% ）poor. There were 11 factors that might affect the prognosis according to the single - factor - analysis. And there were 4 factors, such as the patients＇ occupational status, the compliances, the form of the onset, and their family relationships,that might associate with the prognosis according to the Logest stepwise regression analysis.Condusion：In general,the pmgnesis of the fwst onset schizophrenia is poor, which is affected by bio - psycho - social factors. Some of these factors could be interfered to improve the prognesis.
