二维磁共振波谱分析在评价颅内胶质瘤中的临床应用

摘要 为探讨脑胶质瘤二维磁共振波谱分析(2D1H-MRS)表现及其主要代谢物含量变化,选择120例幕上脑胶质瘤行常规MRI及2D1H-MRS检查,并定量检测其主要代谢物N-乙酰天门冬氨酸(NAA)、含胆碱化合物(Cho)、肌酸(Cr)、乳酸(Lac)和脂质(Lip)含量,采用t检验进一步探讨各级别胶质瘤代谢物含量的差异。结果表明,120例按术后病理结果分为3组:星形细胞瘤(AS)、间变性星形细胞瘤(AA)和胶质母细胞瘤(GM)各40例。随胶质瘤恶性程度的增加,NAA呈下降趋势,Cho呈上升趋势,Cr变化不明显或轻度下降,Lip-Lac呈升高趋势。t检验结果表明,NAA在AS与AA、AS与GM、AA与GM间均具有非常显著的统计学差异(t=13.4,P<0.01;t=56.3,P<0.01;t=7.30,P<0.01);Cr在AS与GM间有显著差异(t=2.30,P<0.05);AS与AA间、AA与GM间无显著统计学差异(t=0.94,P>0.05;t=1.64,P>0.05);Cho在AS与AA、GM间均具有显著的统计学差异(t=4.68,P<0.01;t=9.35,P<0.01),而AA与GM间无显著差异(t=1.56,P>0.05);Cho/Cr在AS与AA、AS与GM、AA与GM间均具有显著差异(t=7.06,P<0.01,t=12.8,P<0.01;t=4.56,P<0.01);NAA/Cho在AS与AA、GM间均具有显著差异(t=18.2,P<0.01;t=20.9,P<0.01),AA与GM也具有显著差异(t=2.11,P<0.05);Lip-lac在AS与AA、GM间均具有显著差异(t=32.1,P<0.01;t=51.7,P<0.01),而AA与GM间无显著差异(t=1.67,P>0.05)。结论:胶质瘤2D1H-MRS表现及其代谢物定量研究有助于胶质瘤临床诊断与肿瘤恶性度分级。
