圆锥型套筒冠义齿咬合重建修复的临床研究

Clinical Effects of Occlusal Reconstruction on Telescopic Dentures

目的:评价圆锥型套筒冠义齿咬合重建修复的临床效果。方法:采用圆锥型套筒冠义齿为32例正常咬合关系丧失的患者进行咬合重建修复,分别于修复前、修复后3个月及修复后3年进行咀嚼效能的检测;检查圆锥型套筒冠义齿内、外冠之间固位力改变,基牙的健康状况以及义齿使用情况。结果:圆锥型套筒冠义齿修复前后咀嚼效能差异有统计学意义(F=7.184,P<0.01);修复3年内患者对义齿的固位效果、舒适度与美观性较为满意。结论:应用圆锥型套筒冠义齿进行咬合重建修复的临床效果满意,咀嚼效能提高;基牙健康与义齿长期的固位情况需进一步临床观察。

Objective： To evaluate the clinical effects on occlusal reconstruction using telescopic dentures. Methods： Thirty-two patients with abnormal occlusion were treated with occlusal reconstruction using telescopic dentures. The masticatory efficiency before and after treatment,the change of the retention of the dentures,and the conditions of the abutment teeth were chosen to be the factors to assess the effects of the treatment. Results： The clinical data showed that the differences of the masticatory efficiency and the retention of the dentures between the stages before and after treatment were significant,but the difference of the retention of the telescopic dentures between the periods of 3 months and 3 years was not significant. Most of the patients were satisfied with the telescopic dentures in comfort and esthetics. Follow-up visits revealed that there were a few abutment teeth showed some problems in the periodontal tissues and/or the hard tissue of the teeth. Conclusion： The telescopic dentures showed good clinical effects in increasing masticatory efficiency. Because of its esthetic feature,good retention and protection to the remained abutment teeth,it is a good choice for the treatment of the occlusal reconstruction.
