关于生理性起搏的几个大型临床试验 被引量:6

摘要 从第1台植入性起搏器问世至今,起搏器发展经历了近50年的历程。在此过程中起搏器的寿命越来越长、体积越来越小、功能越来越强大。从单腔起搏器到双腔起搏器再到三腔起搏器,起搏器的工作方式越来越符合人体生理。随着心脏电生理学和心脏病理生理学研究深入,心脏起搏已不再只局限于提高心率,而力图实现激动顺序和激动时间生理化。以往,单腔心房起搏和双腔房室顺序起搏由于能保持房室的同步性,被认为是生理性的(physiologic);而单腔心室起搏使房室失同步,被认为是非生理性起搏。为了全面评价不同起搏方式之间的差异,20世纪90年代欧美等国家先后进行了多个临床试验,比较了不同起搏方式对心功能正常或心功能不良患者预后影响的差异,其中有代表性包括,CTOPP、MOST、UKPACE及DAVID等试验。这些大规模临床试验证实生理性起搏应能保持房室顺序、能提供频率适应性起搏、起搏后应尽可能保持心室内激动和收缩的同步性。但是起搏器作为一种人工脉冲发生器,尽管目前微电子集成线路已经大大提高其功能,其在模拟生理状态下的心率和心律变化还是有许多不尽人意之处。
