幽门螺杆菌感染慢性胃炎患者血清抗幽门螺杆菌热休克蛋白60抗体表达的意义 被引量:1

Significance of serum antibody to heat shock protein 60 of helicobacter pylori in chronic gastritis with the infection of helicobacter pylori

摘要 目的探讨幽门螺杆菌(Hp)感染患者血清抗幽门螺杆菌热休克蛋白60(Hsp60)抗体与慢性胃炎的关系。方法选择慢性浅表性胃炎及慢性萎缩性胃炎患者作为研究对象,采用14C尿素呼气试验(14C-UBT)及活检组织病理染色检测Hp感染,采用间接固相免疫分析技术测定血清抗幽门螺杆菌Hsp60抗体表达情况,并对胃镜活检标本行病理检查了解胃黏膜炎症情况。结果①慢性浅表性胃炎患者血清抗幽门螺杆菌Hsp60抗体阳性表达阳性率为27.2%,慢性萎缩性胃炎为59.2%。Hp感染慢性萎缩性胃炎患者血清抗幽门螺杆菌Hsp60抗体阳性率明显高于慢性浅表性胃炎患者(P<0.01)。②血清抗幽门螺杆菌Hsp60抗体阳性的慢性胃炎患者胃黏膜Hp密度及14C-UBT测定尿素酶活性均明显高于Hsp60抗体阴性者。③血清抗幽门螺杆菌Hsp60抗体阳性的慢性胃炎患者胃黏膜慢性炎症严重程度和活动性均明显高于Hsp60抗体阴性者。结论血清抗幽门螺杆菌Hsp60抗体可能在幽门螺杆菌感染慢性胃炎的发生、发展中起重要作用。 Objective To investigate the relationship of serum antibody to heat shock protein 60 (Hsp60) of helicobacter pylori(Hp) with chronic gastritis infected with Hp. Methods The patients with chronic superficial gastritis (CSG) and chronic iatrophic gastritis (CAG) were enrolled in the study. Hp infection was detected by 14C urease breathing test and histological staining of biopsy specimens and expression of serum antibody to heat shock protein 60 of Hp by indirect solid - phase immunoassay technique. The conditions of the inflammation of gastric mucosa were studied by pathological examination of endoscopic biopsy spociemens. Results ①The prevalence of the expression of serum antibody to heat shock protein 60 of Hp was 27.2% in chronic superficial gastritis and 59.2% in chronic atrophic gastritis. Prevalence of the expression of serum antibody to heat shock protein 60 of Hp was significantly higher in Hp infected chronic atrophic gastritis than in Hp infected chronic superficial gastritis (P 〈 0. 01 ). ②The density of Hp in gastric mucosa and drease activity detected by 14C urease breathing test were both markedly higher in the patients of chronic gastritis with positive serum Hsp60 antibody than in those with negative antibody expression. ③ The severity and activity of gastric mucosa were both more evident in the patients of chronic gastritis with positive serum Hsp60 antibody than in those with" negative antibody expression. Conclusion Serum antibody to heat shock protein 60 of Hp probably plays an important role in the pathogenesis and development of Hp infected chronic gastritis.
