糖尿病性心肌病的发病机制研究与诊治现状 被引量:4

摘要 糖尿病心肌病是糖尿病(DM)的主要合并症之一,它以心肌肥大、心机纤维化、细胞水平钙转运缺陷、心肌收缩蛋白胶原形成和脂肪酸代谢异常等为特征的一种独特的心肌病变,临床分为亚临床和l临床阶段,前者无任何临床表现,可有左室舒展功能障碍,后者可有充血胜心衰,不伴明显动脉粥样硬化和高血压。糖尿病心肌病的形成与碳水化合物代谢、脂代谢以及钙、钾转运异常有关,也与微血管狭窄、微动脉瘤、心肌肥大、胶原结构紊乱、心肌纤维和血管周围纤维变性等有关。积极预防和治疗糖尿病心肌病可以有效降低DM心血管并发症的死亡率。
