血液透析对眼压影响的研究分析

Investigation of the effect of haemodialysis on intraocular pressure

目的结合该院35例血液透析过程中眼压改变的病例,回顾分析有关血液透析对眼压影响的文献报道,探讨血液透析对眼压影响的机制及眼科检查对于血液透析患者的必要性。方法对该院35例血液透析过程中出现眼部不适的患者进行眼压测定,并回顾分析近40年来的所有同行评议文献。结果对于血液透析对眼压的影响目前仍无统一结论,合并眼科疾病的透析患者眼压受血液透析影响更明显。结论需要进一步的研究探讨血透对眼压的影响机制,合并青光眼等眼科疾病的患者在透析过程中眼压更易升高,有必要进行严密的眼科检查。

[Objective] To review the reported evidence of intraocular pressure （IOP） changes during hemodialysis （HD）, and to analyze 35 cases in our hospital, who were suffered from changes of IOP during HD, in order to review the proposed hypothesis of HD influence on IOP, and to determine if ophthalmic examination is imperative in HD patients. [Methods] IOP of 35 HD patients, with ocular pain during HD, was measured by Goldmann tonome- ter, and the peer-reviewed English literature and all possible relevant articles were selected and analyzed. [Results] The influence of HD on IOP was not clear till now. The possibility of acute IOP rise, in patients with glaucoma during HD, could be much more frequent than in normal patients. [Conclusion] Future studies are needed to clarify the effects of HD on IOP. In patients with glaucoma or with predisposed narrow angles, or eyes with impaired aque- ous outflow, a more strict ophthalmic scheduled examination seems to be feasible.
