经尿道前列腺汽化电切术并发症的防治

Experiences of Prevention and Treatment of Complications Following Transurethral Electrocision and Vaporization of the Prostate in 560 Cases

【目的】探讨经尿道前列腺汽化电切术的并发症和防治措施。【方法】回顾性分析560例经尿道前列腺汽化电切术治疗良性前列腺增生症的临床资料,讨论各种并发症发生原因,并提出其防治措施。【结果】术中大出血8例,尿外渗6例,电切综合征(TURS)6例,排尿困难20例,尿失禁6例,继发性出血6例,尿道外口狭窄20例,反复感染16例,膀胱颈挛缩4例,阳痿(ED)4例,逆向射精18例。【结论】经尿道前列腺汽化电切术是治疗良性前列腺增生症的一项成熟的先进技术;娴熟的操作技术和严密的围手术期处理可以减少并发症的发生和发展。

【Objective】To study the prevention and treatment of complications in transurethral electrocision and vaporization of the prostate.【Methods】The data of 560 patients undergone the treatment of benign prostatic hyperplasia by transurethral electrocision and vaporization of the prostate were reviewed retrospectively,the causes of various complications were discussed,and their prevention and treatment provided.【Results】Of the 560 cases,there were 8 cases of massive hemorrhage,6 cases of urine extravasation and 6 cases of transurethral resection syndrome（TURS）;20 cases of difficulty in urination,6 cases of urinary incontinence,6 cases of secondary hemorrhage,20 cases of urethral external os constriction,8 cases of repeated infection,4 cases of bladder neck contracture,4 cases of inpotence and 18 cases of retrograde ejaculation after operation.【Conclusion】Transurethral electrocision and vaporization of the prostate are mature and advanced technology in the treatment of benign prostatic hyperplasia,skillful operation and appropriate perioperative management would reduce the occurrence and development of complications.
