静脉癌栓对近端胃癌术后生存率的影响

Influence of venous cancerous embolus on the prognosis of patients with proximal gastric cancer

目的探讨近端胃癌的预后因素及静脉癌栓的意义。方法应用Cox回归、χ2检验对有完整随访资料的近端胃癌D2根治术327例患者6项预后因素进行分析。结果静脉癌栓、浸润深度和淋巴结转移均为独立的预后因素。结论静脉癌栓是一种有价值的预后不良信号,阳性者均应进行系统高效化疗。

Objective To investigate the clinical factors on prognosis of proximal gastric cancer,and to explore the effects of venous cancerous embolus in proximal gastric cancer.Methods A total of 327 patients with proximal gastric cancer were treated surgically during a period of 3 years.The relationship were studied of six clinical factors with survival rate of 3 years.Results By Cox regression analysis,χ2 test,depth of tumor invasion,lymphatic invasion and venous cancerous embolus were the independent prognostic factors that measured the survival time.Conclusion Venous cancerous embolus could be used as a predictor of malignant potential and prognosis.Systematic and high-dose chemotherapy after radical operation must be performed to prolong the survival time.
