摘要
目的总结^125I放射性粒子治疗胰腺癌手术体会。方法回顾分析采用胆肠Roux—en—y吻合旁路手术加^125I放射性粒子组织间永久植入治疗的10例晚期胰腺癌的术前准备和手术过程。结果手术顺利，患者术后生活和生存质量得以提高。结论^125I放射性粒子治疗胰腺癌最好采取开腹术式，直视下粒子植入加胆总管-空肠Roux—en—y吻合；术中应用彩色多普勒实时检测粒子植入有利于更好的完成手术前制定的治疗计划；术中加大肿瘤背侧的照射剂量有利于缓解患者腹背部疼痛；手术应先行放射性粒子肿瘤内植入，后做消化道重建。
Objective To summarize the experience of iodine-125 braehytherapy for pancreatic carcinoma. Medthods 10 eases of advanced pancreatic cancer patients who were performed choledochojejunostomy bypass operation combined with intraoperative interstitial ^125I seeds permanent implantation was analyzed retrospectively. Results All operations were successfully performed. The life quality of patients was improved after surgery. Conclusion Laparotomy is the best way to perform ^125I braehytherapy for pancreatic carcinoma treatment plan the operation can be betterly fulfilled with the real time monitor of eolourful ultrasonography during the operation; the increase of radioactive dose in dorsal tumor benefits to the dorsoventral pain relieving; the implantation of ^125I seeds should ahead of the gastrointestinal tract rebuilding during operation.
