摘要
目的探讨胰岛细胞瘤的临床特征、治疗方法。方法对我院1995年1月到2006年1月收治的7例胰岛细胞瘤的临床表现、检查方法和治疗结果进行回顾分析。结果本组经手术切除病理证实胰岛素瘤4例，非功能性胰岛细胞瘤3例。肿瘤位置：胰头1例，胰颈1例，胰头颈交界处1例，胰颈体交界处1例，胰尾3例。7例均施行手术治疗，行胰体尾＋脾切除术1例，胰尾＋脾切除术2例，肿瘤局部切除术4例。功能性胰岛细胞瘤术后血糖上升为5．5～11mmol/L，非功能性胰岛细胞瘤患者术后腹痛症状消失。7例均治愈出院，无手术死亡。结论胰岛细胞瘤主要是胰岛素瘤和无功能胰岛细胞瘤，可行单纯肿瘤切除或胰腺部分切除。
Objective To review the experience on the diagnosis and surgical treatment of pancreatic islet cell tumors. Methods Seven patients with pancreatic islet cell tumor from 1996 to 2006 admitted in tire China-Japan Union Hospital of JiLin University were analyzed retrospectively. Results Seven patients with pancreatic islet cell tumor received operation, which were confirmed by pathology. There were 4 cases classified as insulinomas, 3 cases of nonfimctional pancreatic islet cell tumor.Location of tumor：head of pancreas 1 case, neck of pancreas lease, head-neck junction of panctreas 1 case, neck-body junction of pancreas 1 case and body of pancrease 3 cases. All seven patients underwent operation：resection of body And tail pancreas ＋ splenectomy 1 case, resection of tail of pancreas ＋ splenectomy 2 cases, local resection of tumor 4 cases. Blood sugar increased to 5.5-11mmol/L after operations for insulinoma and abdominal pain disappeared after operation for non-functional pancreatic islet cell tumor. Seven patients were cured and had no operative mortality. Conclusions Insulinoma and nonfunctional pancreatic islet cell tumor are the mostly dominant ones in pancreatic islet cell tumor. It could be performed simply tumor remove or partial pancreas resection for pancreatic islet cell tumor.
