利用混合效应线性模型分析HAART后的CD4+细胞变化 被引量:5

摘要 目的了解接受高效抗逆转录病毒治疗(HAART)艾滋病病人CD4+细胞变化趋势,分析对吸毒感染人群与其他途径感染人群治疗效果的异同。方法描述CD4+细胞在治疗后1年内不同时点上的分布情况;利用混合效应线性模型,拟合CD4+细胞与感染途径、年龄、性别、婚姻状况和检测时间等因素之间的关系。结果CD4+细胞计数与治疗时间和开始治疗时CD4+水平正相关,含3TC方案高于不含3TC方案;吸毒感染人群与其他途径感染人群的免疫学效果不存在统计学差异(P>0.05)。结论接受HAART治疗后1年内,CD4+细胞计数随治疗时间有上升的趋势,且吸毒感染人群坚持HAART治疗,其免疫学效果肯定。 Objective To observationally analyze AIDS patients in regions receiving highly - active antiretroviral treatment (HAART), understand trends of varying CD4 + counts, similarities and differences in a population infected through drug use and those infected through other routes of transmission, and the main contributing factors towards changes in CD4 + counts. Methods The circulation status of CIM + cells at varying time points following treatment was described; using mixed effects model, CIM + cell count correlations with respect to factors such as routes of infection, age, gender, marital status, etc were draw up. Results Statistical analysis demonstrates that CD4 + cell count was positively conrrelated with treatment time and the level of CD4 + at the time of treatmetu; the group of patients infected through drug use showed no statistically significant difference in treatment efficacy from groups infected through other means; CD4 + counts in females were higher than that of males; regimens consisting of 3TC resulted in higher CD4 + counts than those without; and there was positive correlation between baseline CD4 + count and endpoint count. Conclusion Following HAART treatment, CD4 + cell counts show an upward trend with duration of treatment, resembling results from Western reports. In the group infected through drug use, the effectiveness of HAART treatment is definite, thus rationalizing the expansion of HAART treatment across members of this group. Understanding the main contributing factors for changes in CD4 + counts through modeling provides meaningful guidance for clinical practices.
