髁状突骨折和颞颌关节损伤的CT及MRI评价 被引量:1

摘要 目的探讨多层面螺旋CT及MRI对髁状突骨折和关节盘损伤的诊断中的准确性和临床应用价值。方法对25例临床怀疑髁状突骨折的患者,行多层螺旋CT薄层扫描,利用工作站评价髁状突及颞颌关节损伤。其中14例行MRI扫描。结果5例临床怀疑髁状突骨折的患者,多层面螺旋CT三维及多平面重建(MPR)成像清晰的显示了18侧骨折,其中6侧CT考虑关节盘破裂、出血,10侧滑移,6侧关节囊积液。CT阳性的患者及另2例CT阴性但临床症状明显的患者共14例行MRI检查。MRI不仅能显示髁状突的骨折,还能清晰显示关节盘滑移,骨髓水肿及关节表面软骨的损伤。结论多层面螺旋CT三维及MPR可清晰显示髁状突的骨折及关节盘出血;MRI能更好的提供关节盘的滑移,关节表面软骨损伤,骨髓水肿,周围毗邻解剖结构更加详细的资料。
