摘要
目的探讨磁共振弥散成像(DWI)在Wilson’s病病程演变中的临床应用价值。方法对20例临床确诊Wilson’s病患者且常规MRI检查表现为长T1长T2信号组及正常对照组行弥散成像,分析Wilson’s病的表观弥散值(ADC)的变化。结果根据DWI信号,分为高信号、低信号2组,ADC值差异有统计学意义,DWI呈高信号的ADC值降低,DWI呈低信号的ADC值升高。结论ADC值增高可以反映出铜沉积后局部结构疏松,海绵状变性和坏死伴随的髓鞘脱失引起的水分子扩散加快。ADC值减低可以反映出铜沉积后神经细胞变性、肿胀、细胞水肿引起的水分子扩散减低。磁共振DWI在Wilson’s病病程演变中具有一定的临床价值。
Objective To evaluate the clinical value of the MR diffusion weighted imaging (DWI) in the development of Wilson’s disease. Methods MRI, including T1WI, T2WI, DWI were performed in 20 cases of Wilson’s disease proven by clinical diagnoses, and the changes of ADC value were analyzed. Results Two groups were classified into the high signal and the low signal according to the DWI signals. ADC values had significant differences between the two groups, and the ADC values were reduced in the group of the high signal, the ADC values were increased in the group of the low signal. Conclusion The visible increase of ADC values reflects acceleration of water molecule motion owing to spongy softening, cavitation, and demyelination. Visible decrease of ADC values reflects decelera- tion of water molecule motion owing to nerve cell degeneration, swelling, and edema. DWI application reveals the changes of microcosmic structure in Wilson’s disease. MR DWI is of sonce clinical value to Wilson’s disease.
