强直性脊柱炎患者血清与关节滑膜液IL-8、IP-10及RANTES水平测定及其临床意义

IL-8,IP-10 and RANTES in serum and synovial fluid from patients with ankylosing spondylitis

目的探讨IL-8、γ干扰素诱生蛋白10(IP-10)及活化正常T细胞表达和分泌的调节物(RANTES)与强直性脊柱炎(AS)患者病情的关系。方法采用ELISA法检测31例活动期AS患者(观察组)血清、30名健康者(对照组)血清。观察组又按临床症状分为周围型AS(13例)和中轴型AS(18例),检测周围型AS患者关节液IL-8、IP-10和RANTES的水平。结果观察组血清IL-8、IP-10和RANTES水平均高于对照组(均P<0.05)。AS患者关节滑膜液中IL-8和IP-10水平高于血清水平(P<0.05);而RANTES水平低于血清水平(P<0.05)。周围型AS患者血清IL-8水平较中轴型AS患者显著增高(P<0.01)。血清RANTES水平与Bath强直性脊柱炎活动指标(BASDAI)呈正相关(r=0.575,P<0.05)。结论IL-8血清水平可能与AS患者发病形式有关,RANTES可作为病情活动性指标。

Objective To investigate the levels of IL-8, IP-10 and RANTES in serum and synovial fluid from patients with ankylosing spondylitis, and their elinical significance. Methods the levels of IL-8, IP-10 and RANTES in serum from 31 patients with ankylosing spondylitis （experiment group） and 30 healthy suhjects （cnntrol group）, and those in synnvial fluid from 13 patients with ankylositlg spondylitis were determined hy ELISA methods. Experiment group were subdivided into peripheral type and central axial type according to different clinical manifestation. Results Serum levels of IL-8, IP-10 and RANTES in patients with ankylosing spondylitis were significantly higher than those in healthy control group（P〈0.05 ）. In ankylosing spondylitis patients group, the levels of IL-8, IP-10 in synovial fluid were significantly higher than those in senlm, while the level of RANTES in synovial fluid was significantly lower than that in serum （P〈0.05）. Serum level of IL-8 in patients with peripheral type of ankylosing spondylitis was higher than that in patients with central axial type （P〈0.01）.There was a positive correlation between serum level of RANTES and index of BASDAI in patients with ankylosing spondylitis （P〈0.01）. Conclusion Serum IL-8 level can be used for differcntialion of types of ankylosing spondylitis. RANTES may serve as an index of disease activity of ankylosing spondylitis.
