摘要
目的探讨快速成型脊柱模型在脊柱外科初步应用的意义。方法选择8例患者:严重先天性脊柱侧弯4例,退变性腰椎术后翻修3例,腰椎峡部裂1例。应用三维CT断层扫描脊柱,将DICOM数据输入计算机处理,然后进行脊柱病变区域的数字化三维重建和快速成型,并将其应用于临床。结果8例患者按计划完成手术过程顺利,无手术并发症,术后效果良好,术中所见与术前数字化三维重建模型的形状和测量结果完全一致,术后X线复查显示椎弓根螺钉位置正确。结论快速成型脊柱模型这种新技术能够使外科医生在手术前精确和直观地了解脊柱畸形情况,从而有助于指导制定手术计划和手术操作。
Objective To investigate the valuation of rapid prototyping spine model in the surgical planning of complex spinal deformity and degenerative lumbar spine disease. Methods Eight patients inclouded:4 cases of the complicated spinal scoliosis,3 cases revision surgery after lumbar spine operation, 1 case spondylolysis. A three-dimensional CT scan of their spine were performed, and the data were transferred via a DICOM network to a computer workstation. Three dimensional reconstruction and rapid prototype were achieved for clinical treatment. Results Eight patients were successfully operated as surgical planning without any complication,and which bad shown excellent post-operative outcomes. The observed abnormalities in the operation process were the same as the preoperative display and measurement by digital spinal three dimensional reconstruction,and the position of the pedicle screws were accurately evaluated by postoperative X-ray. Conclusion This new method enables the surgeon to appreciate the identical deformity of the spine to be operated from the RP model prior to the real operation,thereby it may help to perform the preoperative plan and surgical procedure readily.
