肾细胞癌的多层螺旋CT诊断及鉴别诊断

Multi-slice Spiral CT Diagnosis and Differential Diagnosis of Renal Cell Carcinoma

目的:分析肾细胞癌的CT表现,提高其诊断和鉴别诊断水平。材料和方法:回顾性分析经手术和病理证实的21例肾细胞癌的多层螺旋CT表现。结果:实性肾癌18例,增强扫描皮质期13例呈明显增强,5例呈轻中度增强,囊性肾癌3例,增强后皮质期囊壁、壁结节、囊内分隔明显增强。实质期及肾盂期增强迅速减退,呈快进快退改变。术前正确诊断率和分期正确率分别为90.5%和80.9%。结论:肾细胞癌多层螺旋CT表现有一定的特征性,大多数能正确诊断。

Purpose：To improve CT diagnostic accuracy and differential diagnostic accuracy of renal cell carcinoma by anaslyzing their CT manifestations.Materials and Methods：Multi-slice spiral CT features of renal cell carcinoma（n=21）,which proved by surgery and pathology,were retrospectively studied.Results：Of 21 cases there were 18 cases with solid renal cell carcinoma and 3 cases with cystic renal cell carcinoma.Thirteen tumors showed marked tumor enhancement in cortical phase,in another 5 cases the enhancement was slight to moderate.Three cystic renal cell carcinomas,presented enhancement of wall with nodules and the partition in cortical phase,but the enhancement faded away in the following parenchymal phase and pyelographic phase.The preoperative diagnostic accuracy was 90.5% and the preoperative staging accuracy was 80.9%.Conclusion：There are some distinguishing CT features of renal cell carcinoma,it can be diagnosed correctively for the most cases.
