COX-2、CX43在复发性原发上皮性卵巢癌中的表达

摘要 目的:探讨环氧化酶-2(cyclooxygenase-2,COX-2)和细胞间隙连接蛋白43(connexin 43,CX43)与上皮性卵巢癌化疗疗效(耐药或敏感)及临床病理的关系。方法:在1999年1月至2003年6月间,从临床资料及随访资料完整的卵巢癌病例中,筛选出60例原发上皮性卵巢癌(primary epithelia ovarian carcinoma,PEOC)和6例正常卵巢(均为宫颈癌切除标本,且术后病理证实双卵巢为正常组织)的存档蜡块,作为研究对象。根据临床诊断分为3组:化疗耐药组30例,化疗敏感组30例,正常卵巢组6例。术前均未接受过放疗,入院后均行广泛性全子宫切除+盆腔淋巴结清扫+阑尾切除+大网膜切除术,全部66例病人均有随访结果。用流式细胞术定量分析66例标本组织中COX-2和CX43蛋白的表达量。结果:CX43蛋白的表达:化疗耐药组<化疗敏感组<正常卵巢组织,单向方差分析两两比较结果均为P=0.000,差异有极显著意义。COX-2蛋白的表达:化疗耐药组>化疗敏感组>正常卵巢组织,单向方差分析两两比较差异均有极显著意义,P<0.005。随临床分期、病理分级、化疗疗程数的增加及有淋巴结转移,CX43蛋白的表达量随之降低,但是差异无显著意义,P>0.05。结论:COX-2蛋白的表达上升和CX43蛋白的表达下降与上皮性卵巢癌的复发、发展及疗程有关,与化疗耐药呈正相关。 Objective: To explore the relations between the expression of CX43 protein ,curative effect of chemotherapy and clinical pathology in primary epithelia ovarian cancer(PEOC) . Methods: To select the paraffin -embedded samples of 60 cases with PEOC and 6 cases with normal ovarian between January 1999 and June 2003. All samples were divided into three groups:drug- resistance group( 30 cases ) , drug- sensitive group (30 cases ) , normal ovarian group(6 cases ,as controls). CX43 protein were detected in 66 samples by flow cytometry (FCM). Results: The levels of COX - 2 : drug - resistance group 〉 drug - sensitive group 〉 normal ovarian group , P 〈 0.005. The levels of CX43: drug- resistance group 〈 drug- sensitive group 〈 normal ovarian group , P 〈0. 001 . Proteins had significant difference between three groups . They had no difference between clinical stage , pathology grade, courses of chemotherapy and lymph node metastases. Conclusion: The levels of COX -2 and CX43 have positive relation to chemotherapy resistance to PEOC, but also have correlation to its recurrence, development and courses of chemotherapy.
