医源性胆管损伤早期处理的重要性

Importance of early management of iatrogenic biliary injury

医源性胆管损伤是胆囊切除的严重并发症,及早发现、正确处理是极为重要的。本文从局部解剖、全身状况及预后等方面详细阐述了胆管损伤早期处理的重要性,并分别对胆管部分损伤、胆总管横断伤、左右肝管横断伤、胆管被结扎等不同伤情的处理进行了探讨。同时介绍了胆管对端吻合、高位胆肠吻合等困难操作的临床经验,对医源性胆管损伤的临床处理提供参考。

Iatrogenic biliary injury is a severe complication of cholecystectomy and its early diagnosis and management are important. In this paper, we elucidated the importance of its early man agement and advanced to respectively explore the methods of management for partial injury of bile duct, transectional injury of total bile duct, transectional injury of the left and right bile duct and bile duct ligation etc. Meanwhile, we introduced the clinical experience in conducting some difficult operations to manage this complication. We believe it would provide a good guideline for management of the iatrogenic biliary injury.
