无透视髋关节镜穿刺的应用

The puncture method of hip arthroscopy without X-ray intensification

目的探讨髋关节镜手术无透视下进行穿刺的方法。方法对40例（50髋）采用髋关节镜无透视穿刺技术。按常规标出穿刺点，穿刺针尾端通过输液管与20ml针管相连，针管内抽入10ml生理盐水。自大转子上穿刺点进针，如针穿入关节，则与之相连的针管内的盐水会被吸入关节腔，或用针管抽吸，有黄色黏稠液体流出。结果穿入髋关节平均手术时间仅为5-10min，效果良好。结论无透视髋关节镜穿刺，可以缩短手术时间，操作简便，减少辐射损伤，无需透视设备，但应有准确的定位和一定的经验及技巧。

Objective To study penetration method of hip arthroscopy without X-ray intensification. Methods 40 cases （50 hips） were undergone joint puncture without image intensification. At routine puncture point superior to greater trochanter, the puncture needle with 10 ml saline was approached into hip cavity. With successful puncture, the saline was sucked or the yellow articular fluid could be pumped out. Results The cost time was 5 - 10 min for puncture, and good result was attained. Conclusions The technique can reduce operating time and radiation, but required accurate orientation and experience.
